Form & Faculty Privacy Policy

    WHAT'S COLLECTED

    F&F collects and retains the names and e-mail addresses of our newsletter subscribers. Newsletter subscribers opt-in through a link provided in the footer of this website. All mailing lists are GDPR compliant. Article 5 of the GDPR requires that personal data shall be:

    a.    processed lawfully, fairly and in a transparent manner in relation to individuals;

    b.    collected for specified, explicit and legitimate purposes and not further processed in a manner that is incompatible with those purposes; further processing for archiving purposes in the public interest, scientific or historical research purposes or statistical purposes shall not be considered to be incompatible with the initial purposes;

    c.    adequate, relevant and limited to what is necessary in relation to the purposes for which they are processed;

    d.    accurate and, where necessary, kept up to date; every reasonable step must be taken to ensure that personal data that are inaccurate, having regard to the purposes for which they are processed, are erased or rectified without delay;

    e.    kept in a form which permits identification of data subjects for no longer than is necessary for the purposes for which the personal data are processed; personal data may be stored for longer periods insofar as the personal data will be processed solely for archiving purposes in the public interest, scientific or historical research purposes or statistical purposes subject to implementation of the appropriate technical and organizational measures required by the GDPR in order to safeguard the rights and freedoms of individuals; and

    f.     processed in a manner that ensures appropriate security of the personal data, including protection against unauthorized or unlawful processing and against accidental loss, destruction or damage, using appropriate technical or organizational measures.”

    In addition to data associated with newsletter F&F occasionally requests feedback on our work from clients and stakeholders through physical and digital surveys. Questions found in these surveys are specific to each project.

    HOW DATA IS USED

    F&F newsletter subscriber data is used exclusively for the purposes of sending newsletters about our work. They are not used for any other purpose.

    Survey data we collect is used internally to improve the outcomes of future projects with similar parameters. It is not used for any other purpose.

    PRESERVING DATA

    To ensure its processing of data is lawful, fair and transparent, F&F maintains a Register of Systems, which is reviewed annually. We may, from time to time, e-mail our subscribers to confirm they would still like to receive our newsletters.

    Any personal data is stored securely using modern software that is kept-up-to-date. Access to personal data is limited to personnel who need access. Appropriate security is in place to avoid unauthorized sharing of information. When personal data is deleted, it is done such that the data is irrecoverable.

    Individuals have a right to 1) access their data and/or 2) have their data deleted. Any such requests shall be dealt with by F&F in a timely matter. To initiate a request, please send an e-mail to hello@formandfaculty.com.

    HOW DATA IS SHARED

    F&F does not sell or share any contact information collected through our newsletter subscription. Additionally, we do not sell or share contact information of clients, partners, vendors, students, or any other stakeholder we engage while working.

    If appropriate, F&F may choose to share select survey findings publicly.  In these instances, no information identifying specific individuals is included.

    In the event of a breach of security leading to the accidental or unlawful destruction, loss, alteration, unauthorized disclosure of, or access to, personal data, F&F shall promptly assess the risk to people’s rights and freedoms and if appropriate report this breach to affected populations and any appropriate regulatory bodies.
